A case of acute erythroid leukemia (AEL) is described in a 75-year-old female patient with cytopenia and bone marrow infiltration with undifferentiated blast cells (?50 %) with decreased bone marrow cellularity. Cytochemical analysis revealed a predominance of a diffusely granular pattern in the periodic acid–Schiff (PAS) reaction, which is atypical for AEL and complicated the cytomorphological diagnosis. The diagnosis was confirmed by immunophenotyping. The case highlights the variability of morphological characteristics in the diagnosis of atypical variants of AEL.
